The Arms and Hands

Note: Arm and hand conditions may have higher ratings for the dominant hand. If you are right-handed, then your right hand/arm is dominant. If you are left-handed, then your left hand/arm is dominant. For the ambidextrous, the rating for the dominant hand will apply regardless of which hand is injured. 


Diagnostic Code 5120: If the entire arm is amputated from the shoulder joint down, it is rated 90% for either arm.


Diagnostic Code 5121: If it is amputated below the shoulder but above the point where the deltoid attaches to the arm, then it is rated 90% for the dominant and 80% for the non-dominant.


Diagnostic Code 5122: An amputation below the deltoid but above the elbow is rated 80% for the dominant arm and 70% for the non-dominant arm.


Diagnostic Code 5123: If the arm is amputated above the insertion of the pronator teres, then it is amputation of the hands rated 80% for the dominant arm and 70% for the non-dominant.


Diagnostic Code 5124: If the amputation is below the insertion of the pronator teres, then it is rated 70% for the dominant and 60% for the non-dominant arm.


Diagnostic Code 5125: If the hand is amputated at a point where it cannot be used at all, then it is rated 70% for the dominant and 60% for the non-dominant hand.


Diagnostic Code 5106: If both hands have been amputated, it is rated 100%.

The Legs and Feet

Diagnostic Code 5160: If the entire leg is amputated at the hip joint, it is rated 90%.


Diagnostic Code 5161: If it is amputated in the upper part of the thigh, then it is rated 80%.


Diagnostic Code 5162: If it is amputated at mid to lower thigh, then it is rated 60%.


Diagnostic Code 5163: If the leg has been amputated but the stump is bad and a second amputation at thigh level is recommended, then it is rated 60%. (This one is a bit odd and is only rarely used.)


Diagnostic Code 5164: If the amputation is at a place around the knee where the knee cannot be used at all, then it is rated 60%.


Diagnostic Code 5165: If the amputation is below the knee, and the knee can still move, then it is rated 40%.


Diagnostic Code 5167: If the leg is amputated at the ankle or in a place so that the foot cannot be used at all, then it is rated 40%.Amputation of the ball of the foot


Diagnostic Code 5107: If both feet have been amputated, it is rated 100%.


Diagnostic Code 5166: If the ball of the foot and toes are amputated, then the rating is 40%.


Diagnostic Code 5170: If all the toes are amputated, not including the ball of the foot, it is rated 30%.


Diagnostic Code 5171: If just the big toe is amputated from the ball of the foot, then it is rated 30%. If the big toe is amputated at a point beyond the ball of the foot, then it is rated 10%.Amputation of the toes


Diagnostic Code 5172: If one or two toes (not the big toe) are amputated from the ball of the foot, then it is rated 20%. If one or two are amputated at a point beyond the ball of the foot, then it is rated 0%.


Diagnostic Code 5173: If two, three, or four toes (not the big toe) are amputated at a point beyond the ball of the foot, then it is rated 10%. If the big toe is one of the toes amputated, then it increases to 20%.

Hand and Feet Combinations

Diagnostic Code 5104: If a hand has been amputated and one foot is not amputated, but cannot be used, it is rated 100%.


Diagnostic Code 5105: If a foot has been amputated and one hand is not amputated, but cannot be used, it is rated 100%.


Diagnostic Code 5108: If a foot and a hand have been amputated, it is rated 100%.
